Pacifist- When Tricky reverts back to normal, let him escape.
True Face of Evil- When Tricky reverts, make sure to PUNCH his face.
Facerape- This one's easy. Make sure to have the chainsaw. When the Magnaguard (the big one) pokes his head into the room, chainsaw his face in half until you get the medal.
